Prior to scheduling an appointment, we request that those seeking counseling complete the following forms and either fax or mail the documents to FLM. FLM Staff will review the documents, pray and assign the appropriate counselor. Please complete the forms as listed below. You will need the free Adobe Acrobat reader to download these forms. Adolescents (ages 5-18): Please fill out the Child/Adolescent Personal Data Inventory form.  Parents should provide answers for children 12 yrs or younger.

Adults (ages 18 and older): Please fill out the Adult Personal Data Inventory form

EVERYONE who will be attending the counseling session must sign and submit a Counseling Advisement and Consent form
